Do I need a physician's referral to see you?

The State of California does not require a physician's referral to see a physical therapist. However, you may need one from your insurance company if you wish to be reimbursed for physical therapy services. Please check with your insurance company. back to top

Who can refer me to OSG?
If your insurance company requires a physician's referral, the State of California defines a primary physician as any of the following: M.D.(medical doctor), D.O.(osteopath), A.C.(acupuncturist), D.C.(chiropractor), D.P.M.(podiatrist), and D.D.S.(dentist). back to top

Will you work with my referring physician?
Absolutely. I will keep them informed of your status though regular reports. They may call me at any time to discuss your situation. back to top

Will you work with my psychologist / counselor?
With your permission, I would be happy to speak with your psychologist or other healthcare practitioner. back to top

Fees for professional services

OSG's fees for treatment are:
$ 55.00 / 1/2 hour session
$110.00 / 1 hour session
$165.00 / 1 1/2 hour session
$220.00 / 2 hour session

OSG's fees for Ergonomic Consultation are:
$180.00 / 1 hour

OSG's fees for Travel are:
$ 25.00 - if less than 15 mile radius
Travel $ 45.00 / hour - if more than 15 mile radius

Payment policy
OSG requires payment at the time of services, rendered in check or cash, unless previous arrangements have been made. back to top


Financial agreement
OSG requires a signed financial agreement prior to initiating treatment. This agreement simply states that you will be responsible for the payment in full of fees for services rendered. back to top


Does OSG accept credit cards?
At present, OSG does not accept credit cards. back to top


Cancellation policy
Clients must cancel 24 hours in advance or be subject to the full appointment fee. back to top


No show policy
A client who fails to cancel or come to their appointment is subject to the full appointment fee. back to top


Late arrival policy
Please do your best to be on time. You will be responsible for the full session fee regardless of your time of arrival. If, in spite of your best efforts, you will be late, please call On Solid GroundSM to notify us. We will accommodate you if possible. back to top


Childcare policy
Logistical conditions do not allow us to provide childcare. Please arrange for child-care prior to your arrival. back to top

What insurance plans does OSG participate in?
Currently, OSG is making application for participation in several insurance plans. Please inquire. back to top


Will OSG bill my insurance company?
Not directly. OSG will provide you with a receipt which you may submit to your insurance company for reimbursement. back to top


How much will my insurance company reimburse for OSG's services?
Insurance companies vary regarding reimbursement. Please contact them. back to top


Do you accept Worker's Compensation Insurance Claims?
Yes, OSG accepts Worker's Compensation Claims. However, you need to be authorized by your claim adjustor prior to scheduling your first visit. back to top


Does OSG accept insurance for car accidents?
Yes, OSG will accept payments designated as MedPay. As a rule, OSG does not accept third party payments. If this is your situation, OSG requires payment at the time treatment is provided. You will be reimbursed for physical therapy services at the time of your settlement. back to top

As a rule, OSG does not accept liens. Please call to discuss your situation. back to top

OSG is located in the Medical Arts Building at 2000 Van Ness Avenue, Suite 333, San Francisco, CA 94109. The Medical Arts Building is on the northeast corner of the Van Ness Avenue / Jackson Street intersection. back to top


Directions

Driving - from the North Bay
From 101 South: cross the Golden Gate Bridge, take the 101 South-Downtown Exit. Exit at the next 101 South-Downtown Exit. Continue on Richardson Avenue until you turn left onto Lombard. Continue on Lombard to Van Ness Avenue. Turn right onto Van Ness Avenue. Turn left onto Broadway. Turn right onto Polk and go two blocks. Turn right onto Jackson. The side entrance to 2000 Van Ness is on Jackson, before Van Ness Avenue. back to top

Driving from the East Bay
From 80 West: cross the Bay Bridge and stay in the right hand lane. Take the first exit - Fremont Street. At the bottom of the hill turn left. Proceed on Fremont in the middle lane until you cross Market Street. Travel one block, turn left onto Pine Street. Proceed on Pine Street to Larkin Street. Turn right onto Larkin (a one-way - north). Stay on Larkin Street, cross California Street (where Larkin becomes two-way), travel 4 blocks, turn left onto Jackson, cross Polk Street. The side entrance to 2000 Van Ness is on Jackson, before Van Ness Avenue. back to top

Driving from the South Bay
From 101 North: take the Ninth Street-Civic Center Exit. Turn left, then continue on Ninth Street in one of the right hand lanes. Be in a lane marked Larkin Street before you cross Market Street. Cross Market Street. Stay on Larkin Street ( a one-way north), cross California Street (where Larkin becomes two-way), travel 4 blocks, turn left onto Jackson, cross Polk Street. The side entrance to 2000 Van Ness is on Jackson, before Van Ness Avenue. back to top

By Muni
The Muni bus lines 27 Bryant and 42 Van Ness stop at 2000 Van Ness Avenue. The Muni bus line 1 California stops at Clay and/or Sacramento and Van Ness Avenue, 2 - 3 blocks south of Jackson. back to top

The in-bound Golden Gate Transit stops at Pacific and Van Ness, 1 block north of Jackson. The out-bound Golden Gate Transit stops at Broadway and Van Ness, 2 blocks north of Jackson. back to top

Street parking
There is metered parking for 1/2 hour and 1 hour along Van Ness Avenue and in the immediate area. There is 2 hour parking on Jackson west of Van Ness Avenue and along Franklin and Gough Streets. back to top


Parking garage
There is a parking garage at 1650 Jackson, between Polk and Van Ness Avenue, less than 1/2 block from the Medical Arts Building. Call for other parking facilities in the neighborhood. back to top


Disability parking
